Client Needs Assessment
Used to understand a client’s situation and care needs before placement, including decision makers, current living situation, move timeline, and the level of care being sought.
Client Agreement
The agreement between Golden Heart Senior Placement and the client, or their authorized representative, that outlines our placement services, authorizes us to share information with communities, and discloses how we are paid. There is no fee to the client.
Client Financial Assessment
Used to review a client’s financial picture before placement, including financial decision makers, monthly income, care budget, private pay runway, and insurance coverage such as Medicare and long-term care.
Community Forms
Referral and Placement Agreement
The agreement between Golden Heart Senior Placement and partner senior living communities (facilities and group homes) that defines our referral services and sets the referral fee terms under Nevada law.
